WHAT IS A TRANSITION IN CARE?

The term "care transitions" refers to the movement service user make between health and social care practitioners and settings as their needs change They can be both short and long term, for example, acute hospital admissions, foster care placements, transitioning from care to adulthood.

There are 7 stages of transition DENIAL, ACCEPTANCE, DEPRESSION, NUMBNESS, INTERNALISATION, TESTING, SEARCHING FOR MEANING*******Hopsin et al 1992

LINK BETWEEN ENVIRO AND IDENTITY:-

For many home is a base that provides stability and a secure base, it is a social enviro and the everyday familiarity offers a sense of security (in this way can be seen as a psych enviro). GOOD PRACTICE IN TRANSITIONS:-

Give as much info in advance
S'one to talk to independant of the services (?advocat)
Advanced visit if possible
Having a say in what happens, retaining some control

GOOD PRACTICE IN REGARDS TO LOOKED AFTER CHILD:-

Children's Act 1989 advises introductory meeting between child and foster carer
Hedy Cleaver study 2000 showed this didn't alwats happen and re enforced the need fro this good practice to take place as children between the ages of 5-12 often haven't stayed away from home. JORDAN, block 2

OTHER THINGS TO REMEMBER:-
Young people transitioning frm care to independance need support as it a time of gain; thier independance, and loss = friends and relationships with their S/W and/or their foster carers
Many transition into unsuitable accommodation, a study showed that it was in fact 42%. This leads to feelings of worthlessness, isolation and marginilisation
The physical appearance of a building is very important in making a transition run smoothly.
