    1. NRTIs

      Nota:

      • abacavir (ABC) & lamivudine (3TC) are both NRTIs. The former is also used for HBV tx.
      1. INSTI

        Nota:

        • dolutegravir (DTG) is the integrase inhibitor 
        1. Tablets ONLY
          1. 600 mg abacavir
            1. 50 mg dolutegravir
              1. 300 mg lamivudine
            2. Combination Anti-Retroviral
              1. HIV Treatment
                1. helps prevent resistance

                  Nota:

                  • HIV is a fast adapting virus. Using multiple drugs with different MOAs helps prevent resistance from developing to a single agent.
                2. Indications
                  1. HIV-1 infection

                    Nota:

                    • 1T QD
                    1. adult & pediatrics > 40 kg

                      Nota:

                      • if used in combination with UGT1A or CYP-3A inducers, an additional 50 mg of dolutegravir should be taken 12 hours after Triumeq
                    2. Monitoring
                      1. LFT

                        Nota:

                        • many HIV drugs can adversely affect the liver
                        1. CD4+

                          Nota:

                          • Monitoring the progress of the disease
                        2. Contraindications
                          1. concommitant dofetilide
                            1. Severe Hepatic Impairment
                              1. HLA-B*5701
                                1. 1st trimester of pregnancy
                                  1. St, John's Wort
                                  2. ADEs
                                    1. hypersensitivity

                                      Nota:

                                      • severe rash, fever, malaise, fatigue, muscle/joint aches, blisters, conjunctivitis, facial edema, hepatitis, angioedema 
                                      1. lactic acidosis
                                        1. liver toxicity
                                          1. immune reconstitution syndrome
                                            1. Most Common
                                              1. sleep disturbances
                                                1. tiredness
                                                  1. headache
                                                2. Patient Consultation
                                                  1. Calcium, Zinc, or Iron containing products

                                                    Nota:

                                                    • take 2 hours before or 6 hours after  -includes antacids
                                                    1. do NOT double dose

                                                      • if dose is missed, take ASAP unless too close to next dose
