Absolute duty An absolute duty is something that must be complied with
Accident An undesired event that results in injury, ill-health or property damage
Accident Injury Rate (AIR) Most commonly used accident rate. Expression of the number of defined accidents in a given period per thousand employees.
Accident Frequency Rate (AFR) Accident Frequency Rate (AFR). The frequency rate is the number of defined accidents in a period per 100,000 person hours
Accident traingle A visual way of showing how unsafe events in the workplace will generally lead to near misses but that a proportion will also also result in accidents
ACoP Approved Codes of Practice (ACoP) give practical advise and offer examples of good practice on how to comply with the law
Active failures (re. human failure) Active failures - immediate consequence made by front-line people such as drivers, control room staff or machine operators

Auditing Structured process of collecting independent information on the efficiency, effectiveness and reliability of the total health and safety management system, and includes drawing up plans for corrective action
