Disminución de insulina circulante
Aumento leve de Hormonas contrareguladoras (Glucagón, catecolaminas, cortisol y HC)
incapacidad de la glucosa de entrar a hígado, músculo, tejido adiposo
Manifestaciones clínicas
síntomas
Malestar general,Poliuria, polidipsia, polifagia, pérdida de peso, fatiga, dolor abdominal
Exploración física
Hipotensión,pobre turgencia de la piel Respiración de kussmaul, alteración del estado mental , choque, taquicardia, inicio isidioso
Diagnóstico
Glucosa elevada siempre presente
Tratamiento
Líquidos
solución salina al .9/ velocidad 15-20 ml/kg/h 1-1.5 L PRIMER HORAS
Glucosa en 200 mg/dl cambia solución por una mixta: glucosa al 5%- salina .45%
Insulina
Bolo IV de .1UL/Kg de insulina rápida, disminución de glucemia 50-70mg/100ml/h
potasio
Pacientes con menos de 3.3 MEq/L, NO insulina dar 20-30 mEq/hr hasta que el K sea mayor de 3.3 mEQ/l
Bicarbonato
Diagnóstico deferencial
Cetoacidosis alcohólica, estado hiperosmolar hiperglucémico
Criterios de resolución
Glucosa menor a 200mg/dl, Bicarbonato sérico mayor a 18mEq/l y pH venoso mayor o igual a 7.3
