Choque hipovolémico:
Cuando el volumen intra vascular se agota como resultado de:
Hemorragia.
Pérdida GI.
Pérdida urinaria.
Deshidratación.
Tercer espacio:
Quemadura.
Traumatismo.
Pancretitis.
Disminuye precarga y GC --> aumento de resistencias vasculares por vasoconstricción compensatoria.
Triada:
Acidosis.
Hipotermia.
Coagulopatía.
Tx:
Mejorar aporte de O2.
Manejo de etiología.
Bolo 500-1000 ml de cristaloide en 30min.
300ml de solución por cada 100 ml perdidos.
SIEMPRE cuidar uresis.
Aminas:
Dopamina 2-3 microg/kg
Para pacientes con falla cardiaca.
Dobutamina 2.5-20 microg/kg.
Hasta 10 para ver efectos.
Antes de pasar aminas la PVC debe de ser de 10-12, si no no funcionará el tratamiento.
Clínica:
Pálido, frío, taquicardia, diaforesis, hipotenso.
