The case study is in a medical practice with two patients in the surgery with same names different spellings. Date of births are within the same time frame one born in 1954 and the other person 1955. Patients have had the same tests preformed and saw the same Doctors. Patients documents have been finalised and put into patient files. One patient had a clear result the other patient had a positive result which means he needed an angiogram to clear the blockage.
