      2. RELATION TO SHINGLES
        1. Herpes zoster virus is VZV that reactivates later in life to cause shingles.
        2. VACCINE
          1. live attentuaed varicella virus
          2. TRANSMISSION
            1. from person to person by directly touching the blisters, saliva or mucus
              1. The virus can also be transmitted through the air by coughing and sneezing.
              2. STAGES
                1. successive and can be seen together
              3. PATHOGENESIS
                1. SYMPTOMS
                  1. pain/ burning
                    1. fluid-filled blisters
                      1. red rash
                        1. sensitivity to touch
                          1. itching
                            1. unilateral, doesn't cross midline usually
                            2. RISK FACTORS
                              1. age >50 y.o.
                                1. diseases that weaken immune system, e.g. cancer, aids
                                  1. undergoing radio or chemotherapy
                                    1. certain medication use, e.g. prednisone
                                    2. COMPLICATIONS
                                      1. vision loss
                                        1. skin infections
                                          1. neurological problems
                                            1. POSTHERPETIC NEURALGIA
                                              1. severe pain that continues for more than 1 to 3 months in the same place that the shingles occurred, even after the rash goes away. 8-70% of cases
                                            2. TREATMENT
                                              1. PAIN MANAGEMENT
                                                1. narcotics, e.g. codeine
                                                  1. numbing agents, such as lidocaine, delivered via a cream, gel, spray or skin patch
                                                    1. capsaicin topical patch (qutenza)
                                                      1. an injection including corticosteroids and local anesthetics
                                                      2. PREVENTION
                                                        1. SINGLES VACCINE: for people aged 60 or over, or 50 or over for people living with someone who is immunocompromised.
                                                        2. ANTIVIIRALS: - acyclovir, valacyclovir, and famciclovir
