- 1Congestive Heart FailureThe heart fails when it is unable to eject blood delivered to it by the venous system.
Most common hospital adminsion diagnosis for persons >65 years old.
Left sided heart failure, most common type.
Right sided heart failure
Biventricular heart failure.
High output heart failure. (least common heart failure)
Blood builds up behind the failed heart
In LHF, blood backs up in the lungs (pulmonary congestion)
In RHF, blood builds up in the systemic venous system. - 2Left sided heart failureLV cannot efficiently eject blood into the aorta.
Causes an increase in the LVEDV and LV end-diastolic pressure.
Increased LVEDV and LVEDP leads to a backup of blood into the lungs producing pulmonary edema.
Decrease in ventricular contraction
Decreased LV contraction defines systolic heart failure.
SHF is the most common type of LHF.
Causes of SHF include
Ischemia, due to coronary artery atherosclerosis (most common cause)
Post-myocardial infarction (MI), myocarditis, and dilated cardiomyopathy.
Noncompliant LV (stiff ventricle) with impaired relaxation
Noncompliant LV with impaired relaxation defines diastolic heart failure.
In DHF, there is increased LVEDP.
Causes of DHF
Concentric LVH due to essential HTN is the most common cause of DHF.
Other causes include AV stenosis, hypertrophic cardiomyopathy, and restrictive cardiomyopathy(amyloidosis or glycogenosis)
