1. CHECK 1: when the medications are pulled or retrieved from the automated dispensing machine or the medication drawer

   • 1) Check that med is for the right patient
   • 2) Check that it is the right medication
   • 3) Check the order and confirm that it is the right dose
   • 4) Check the order and the appropriateness of the route ordered
   • 6) Check that you are giving med at the right time
   • 7) Check the frequency of the medication ordered
   • 8) Check the reason why this medication was ordered
   • 5) Check the appropriateness of the site of administration ordered

2. CHECK 2: when preparation of the medications for administration takes place.

   • 4) Check the order and the appropriatness of the route ordered
   • 3) Check the order and confirm that it is the right dose
   • 7) Check the frequency of the medication ordered
   • 2) Check that it is the right medication
   • 8) Check the reason why this medication was ordered
   • 5) Check the appropriateness of the site of administration ordered
   • 6) Check that you are giving med at the right time
   • 1) Check that med is for the right patient
   • 9) Pour the medication into the medication cup

3. CHECK 3: At the patient's bedside just before medications are given. This is also an outstanding opportunity to teach the patient about the medications.

   • 3) Confirm that it is the right dose
   • 2) Check that it is the right medication
   • 6) Conform that medication is administered at the right time
   • 4) Confirm site of administration
   • 8) Confirm the reason why medication is being administered
   • 1) Check that medication is for the right patient using two patient identifiers
   • 5) Confirm the route of administration
   • 7) Confirm the frequency ordered
   • 9)Teach the patient about the medications and ensure that the patient takes the medication
   • 10) Document administration AFTER giving the ordered medication
