1. Prepare

   • Blood group and cross match
   • Consent

     • 10 rights
     • legal order (blood IVT form
       • Blood product time, Time, date, Pt ID, signature, Drs name..
   • Educate (reason, reactions)
   • vital signs, rash, stable?
   • IV site patent, NAD
     • resit if necessary
   • Equipment
     • Blood giving set 200 micron filter
     • NS
     • Pump
     • Check suction
   • Business hours
   • toilet patient
   • consider work load, prioritise

2. Check

   • Name, DOB, URN, address (ID band and verbal confirmation)
   • Blood bag
     • Front
       • expiry
       • blood group
         • Rh
       • product i.e. packed cells
       • volume
       • pack number, batch number
     • Back
       • pt details
       • blood type (patent and pack)
       • batch number
   • Blood form again
   • viability of bag - clots, purple colour
   • Paper work
     • Prescription
     • pathology form
   • consent
   • 2 nurses
     • 1 RN
     • independent

3. Give

   • Prime line with saline
   • monitor patient for adverse reactions
   • 15 minutes - RN stays
   • Obs as per policy and procedure
   • discard clinical waste
   • 30 minutes
     • start within 30 minutes
     • return within 30 minutes
   • give within 4 hours - cease after 4 hours
   • infection control

4. Documentation

   • progress notes
   • FBC
   • Sign blood order
   • pathology form
