  1. 1Anterior Drawer TestSupine, knees bent at 90, examiner stabilizes foot with hip and places thumbs over a. knee joint line & pulls the tibia anteriorly.
    Px: a. cruciate ligament sprain
    Excessive motion: a. cruciate rupture
  2. 2Posterior Drawer TestSupine, knees at 90, examiner places thumbs over the anterolateral & anteromedial joint line & pushes the tibia posteriorly
Px or increased motion: PCL sprain or rupture
  3. 3Apley's Compression TestProne, knee flexed 90, examiner applies downward force on the foot & rotates internally/externally
Px or crepitis, relieved by knee distraction: Meniscal damage
  4. 4Apley's Distraction TestProne, knee flexed 90, examiner stabilizes patient's thigh by placing knee over patient's thigh, applying long axis distraction (pulling up) & intern. & extern. rotates leg
Px w/ distraction: capsule/ligamentous lesion, collateral ligament sprain. Decreased Px following distraction: Meniscal lesion
  5. 5Patellofemoral Grind Test (Clarke's Sign)Examiner compresses the patella against the femur, moving it lat., med., sup., inf., & in a twisting motion (twist and grind)
Px/Crepitis: Chondromalacia patella, DJD, Osteochondritis of patella, or fracture
  6. 6Clarke's Test ModifiedExaminer compresses the quad mm. 2cm above the sup. pole of the patella, the patient is then instructed to contract the quad mm.
Retropatellar Px (behind the knee): Chondromalacia patella, degeneration of patellofemoral joint
  7. 7Lachman's Test (Gold standard for the evaluation of ACL & PCL damage)Supine, knee bent 15-30
Lachman's Anterior: examiner stabilizes the patient's femur with one hand & then pulls the tibia anteriorly with the other hand
Lachman's Posterior: same procedure except repeated with the examiner pushing the tibia posteriorly
Px with normal a. translation, Px with increased a. translation: ACL sprain, rupture
Px with normal p. translation, Px with increased p. translation: PCL sprain, rupture
  8. 8McMurray's TestSupine, hip & knee flexed 90, examiner stabilizes patients distal quad mm. & grips patient's heel with other hand & applies long axis compression; rotating tibia INTERN. with VARUS force & then rotating the tibia LATerally, with VALGUS force
    Px/Crepitis: Meniscal lesion
    Valgus/EXTERN: Lateral Meniscal
    Varus/INTERN: Medial Meniscal
  9. 9Nobel Compression TestSidelying, examiner applies l. to m. pressure over later epicondyle w/ thumb & slowly flexes & extends the leg (3-4 times)
Px or palpable tendon snapping = Iliotibial band syndrome
  10. 10Patellar Apprehenson/ Fairbank's TestSupine, pushing patella laterally & observes the patient for signs of verbal & nonverbal apprehension or reflex quadriceps contraction
Apprehension or relfex: patellar instability, subluxation, tracking disorder, dysfunction
  11. 11Patellar Ballottement TestExaminer applies a. to p. pressure over the patella & gently oscillates the patella, palpating for motion
Increased motion or spongy joint feel: intra-articular knee swelling
  12. 12Patellar Sweep Test (Bulge Sign, Brush Test)Sweep hands down the patient's quad mm. in a "milking" motion in an attempt to force fluid to gather around the patella
Excessive fluid end or patellar elevation: knee swelling/edema
  13. 13Posterior Sag Sign (Gravity, Godfrey Test)Supine, knees 90, hips 45, examiner compares prominence of tibial tuberosities bilaterally
Sag back on the femur: PCL tear (posterior translation knee instability)
  14. 14Q Anglesad
  15. 15Reverse Lachman'sProne, examiner pulls the tibia p.
Px with normal trans.: PCL sprain
Px with excessive movement: PCL rupture
  16. 16Retreating MeniscusPalpates the medial joint line a. to m. collateral ligament & with opposite hand rotates the leg internally & externally by twisting the ankle observing for the presence or disappearance of the meniscus
Meniscus does not disappear while rotating the leg: meniscal lesion, tear
  17. 17Q-Angleone line drawn from ASIS through midpoint of patella, the other line drawn from tibial tuberosisty through midpoint of the patella. Q-Angle is formed by the intersection.
<13 deg: genu varum (bow leged), LCL
    >18 deg: genu valgum (knock knee), MCL
  18. 18Varus Stress TestStabilize l. ankle & applied m. to l. force at knee (push knee out). Repeat with the knee slightly flexed at 25 deg.
    Px: LCL strain
    Increased motion: LCL rupture
  19. 19Valgus Stress TestStabilize m. ankle, apply force l. to m. at the knee (push the knee in). Repeat with the knee slightly flexed 25 deg
    Px: MCL sprain
    Increased motion: MCL rupture
  20. 20Tinel's Sign (of the knee)Tapping of the knee, which tests the integrity of the saphenous nerve.
    Tingling, Px: compromised saphenous nerve.
