A person acting responsibly for the client. - 17SAME APPLICATION CAN BE USED WHEN:*case closes and reopens same calendar month
*adding newborn
*case suspended for one month because of ovi and reopened the month after the suspended month - 18IF ADDING A PERSON TO CASE OTHER THEN A NEWBORN:use a new application form or add addendum to the current application
- 19WITHDRAWAL OF APPLICATION OR NON COMPLETION OF THE APP PROCESSby 45th day , app is considered withdrawn if does not complete the process by (DOR)
