Basic resuscitatión

  1. Pharmacotherapeutic treatment

    • vasoconstrictors
      • epinephrine
      • vasopressin
      • Improve coronary perfusion pressure
    • Antiarrhythmics
      • lidocaine
      • amiodarone
      • magnesiun sulfate
      • They are administered to control and prevent additional episodes of arrhythmias
  2. conceptualization

    • cardiorespiratory arrest
      • it is almost always a consequence of previous deseases of the heart that electrically produce a ventricular fibrillation (vf) that is a chaotic rhythm
      • It is the global cessation of circulation (cardiac mechanical activity) in an individual in which his death should not be expected at the time and is diagnosed by the absense of neurological response
        • Does not respond to the call
        • Does not move
        • Does not breathe
        • it has not pulse
    • Respiratory faliure
      • it is absense of respiratory movements (apnea) but the pulse is present
    • Clinical death
      • Apnea plus total cardiac arrest
    • cardiac death
      • To stablish this condition it is necessary to detect an intractable electrical asystole for at least 30 minutes despite advanced resuscitation
  3. The general objetive of basic resuscitation is for the patient survive neurologically intact

  4. Short-term objectives

    • patient stabilization
    • achieve adequate cerebral blood flow
    • achieve adequate coronary perfusion pressure
  5. chest compression

    • 30 compressions and 2 breaths should be done by a CPR cycle
    • if oxigen is aviable, it should be used as a supplementary
    • changed every 2 minutes of reanimation
    • 100-120 per minute to a depth 5-6 cm
    • Rhythmic downward pressure with the heel of the hand
  6. Basic cardiopulmonary resuscitation (RCPC)

    • Steps
      • A-A airways
        • A1 open the airways and assess their permeability
        • A2 cleaning and unblocking if necessary
      • B-B breating or ventilation
        • B1 evaluate breathing (MES meneuver for 10 seconds)
        • B2 give 2 rescue breaths to check permeability and oxygenate
      • C-C circulation
        • C1 evaluate circulation (central pulse for 10 seconds
        • C2 start heart compressions if there is no pulse, rhythm and frequency according to age
  7. After the reanimation

    • Monitor oxygen levels and intracranial pressure
    • Make an electrocardiogram (ECG)
    • Blood pressure should be supported if necesary (with replacement of fluids and inotropic drugs) at a systolic pressure >100 mmhg (or average pressure of 65-80 mmHG)
