1. Primary Care

   Annotations:

   • Service provided by physicians (also called PCP), during the very first encounter with a patient seeking medical care. Usually is a general practioner
2. Secondary Care

   Annotations:

   • services provided by medical specialists referred by PCP (Primary Care Provider).   
3. Tertiary Care

   Annotations:

   • Services provided by specialist hospitals or regional centers. Highly specialized medical care that involves advanced and complex procedures and treatments performed by medical specialists in state-of-the-art facilities

1. Physician clinics

   Annotations:

   • dedicated to OP for diagnosis & treatment.   
2. Physician group practices

   Annotations:

   • "small rural IDN" - a formal association of 3 or more physicians 
3. IDNs

   Annotations:

   • Integrated Delivery Network: Network of providers giving coordinated continuum of services. (ie. Kaiser Permanente) Health plan enters into a contractual arrangement with specified providers.  
4. DC centers

   Annotations:

   • Day Care centers – "Less than 24 hrs" facilities performing procedures on an outpatient basis   
5. Diagnostic Centers

   Annotations:

   • "Tertiary Care" type offer various modalities of testing ranging from cardiac, radiology, laboratory, nuclear imaging, etc.   
6. Skilled Nursing Facility

   Annotations:

   • Nursing home - (kind of like a retirement home but with professional medical services offered)  offers constant nursing care to patients with physical/mental disabilities   
7. Hospice Care

   Annotations:

   • Hospice care - "end of life" is a type of care that focuses on the palliation of a chronically ill, terminally ill patient's pain ,symptoms and addressing their emotional and spiritual needs.
