RADS: The Reactive Airways Dysfunction Syndrome
Pneumoconiosis
accumulation of dust in the lungs and the tissue reactions to its presence
tissue reaction may be non-collagenous( minimal stromal reaction) or collagenous(when scarring is permanent)
Characterised by fibrosis & caused by chronic inhalation of mineral dusts
lungs cannot expand- dyspnea
Sign & Symptoms
dyspnea, Shortness of breath, Chest pain
Asbestosis
Diffuse fibrosis caused by a persistent alveolar inflamation
Irregular opacities predominantly in the lung bases
Clubbing is common
Silicosis
Simple Silicosis
small nodules, predominantly in upper lobes; patient often symptomatic
Complicated Silicosis (Progressive Massive Fibrosis)
coalescence into large nodules or masses with retraction of upper lobes
Coal Worker’s Pneumoconiosis (CWP)
Can cause industrial bronchitis, emphysema and progressive massive fibrosis
Xray looks worse than patient
Simple CWP
asymptomatic
small rounded opacities predominantly in the upper lobes
Complications
Caplan’s Syndrome
syndrome with rheumatoid arthritis features, PMF, and usually (>70%) a positive rheumatoid factor
Hard Metal Disease
Cobalt
metal cutting or grinding tools and in jet engines turbines blades
