Pregunta Respuesta
Define cardiogenic shock Diminished cardiac output leading to impaired tissue perfusion Most extreme form of pump failure
How much of the ventricle must infarct for cardiogenic shock to occur? 40%
What is the mortality of cardiogenic shock? >85% with treatment
Signs and symptoms of cardiogenic shock - ALOC - Confusion, restlessness, anxiety, stupor, coma - Poor perfusion - cool, clammy skin, pallor - Weak or absent extremity pulses - Tachycardia - Slow or absent cap refil - BP <90 systolic or >30mmHg below normal
Cardiogenic shock confounding factors - Arrhythmias - Hypovolaemia - Decreased vascualr tone
Treatment of cardiogenic shock - Priorities are: - Rate - Rhythm - BP, volume, pump, vascular tone - Improve oxygenation and peripheral perfusion - Avoid increased cardiac workload/myocardial oxygen demand
Cardiogenic shock management, more detail - Keep pt supine unless dyspnoeic, do not elevate lower extremities - Oxygenate via NRB - Consider assisting ventilations, decrease work of breathing - Monitor ECG - IV TKVO, limit fluids unless RHF, IV morphine - CCP to correct major rhythm disorders
