Pathogenesis
1. Infarction
  1. Appears darker on imaging - hypodense
  2. Ischaemic injury
    1. Focal cerebral infarction
    2. Global injury; ischaemic encephalopathy
  3. Causes
    1. Thrombus
    2. Embolus
    3. Vasospasm
    4. Herniation
    5. Local vasculitis
    6. Poor perfusion without acute obstruction
  4. Thrombotic infarcts
    1. Most commonly occur in middle cerebral artery territory
    2. Thrombosis superimposed on atheromatous plaques in extracranial or large intracranial arteries
  5. Molecular events of cerebral ischaemia 1. Decreased neuronal ATP production 2. Altered ion transport (increases in intracellular Na and Ca and in extracellular K) 3. Increase in Na and water causes swelling and depolarisation of the neuron 4. Release of exocitatory neurotransmitters 5. Activation of NMDA receptors 6. Phospholipase activators cause free radicals 7. Damage to cell membrane, proteins and DNA 8. Liquefactive necrosis

  4. Subarachnoid
  5. Intracerebral
  6. Causes raised intracranial pressure
Management POST
1. Cholesterol lowering
  1. Statins
2. Antithrombotics
  1. Antiplatelets; long term, e.g. aspirin
  2. Anticoagulants; e.g. warfarin
3. Stroke unit care (see medical management during for list of who is involved in the stroke team)
4. Cardiac investigations
5. Lifestyle
  1. Stop smoking
  2. Increase regular physical activity
  3. Reduce excessive drinking
  4. Eat a healthy diet
6. Blood pressure control; extremely important long term
7. Stroke rehabilitation
