Question Answer
Definition Of Personalisation Recognising that the person has individual strengths, preferences, wishes and aspirations. This means putting the person in the centre of care.
Personalisation To An Individual Means... - Independence. - Empowerment. - Control. - Addressing needs. - Participation. - Preferences. - Meeting aspirations. - Choice.
Key Features Of Personalisation - Personal budgets. - Co-production. - Choice and control. - Self-assessment of needs. - Changing role of professionals.
Definition Of Personal Budgets The amount of money the local authority allocates for your care, based on its assessment of your needs. You can be put in charge of this budget either by telling the local authority how you would like it spent (managed accounts), or by the council giving you the money so that you can directly pay for your own care (direct payments).
Personal Budgets - Direct Payments These give you the most control over your care, and mean that if you are unhappy with the services you're getting, you can decide to change who gives you the care services without the process of going through the local authority.
Personal Budgets - Managed Accounts. This money is kept with the local authority and pays for the care a person is receiving, this can be state funded or private services.
Coproduction/Citizenship Model This means that there is a collaboration/joint planning with care workers and the people receiving care to get the best outcome.
Coproduction In Action 1) Older people are involved throughout. 2) Older people feel safe to speak out and are listened to. 3) We work on issues that are important to older people. 4) It is clear how these decisions are made. 5) Older people's skills and experiences are used to achieve change. 6) Meetings, materials, and venues are accessible. 7) Take action. 8) Progress is evaluated.
Choice And Control Personalised approaches such as personal budgets involve enabling people to identify their own needs and make choices about how and when they are supported to live their lives. People need access to information, advocacy and advice so that they can make informed decisions.
Choice And Control - Where Care Is Provided... The ability to make choices about how people live their lives should not be restricted to those who live in their own homes.
