HPC - itch, pain, mucosal involvement (eyes, mouth, genitalia), site of onset (flexural, extensor, dermatomal distribution), associated symptoms (malaise, fever), joint symptoms
PMHx Atopy, Eczema, Psoriasis
PSHx excision of skin cancers
DHx New medications over last 6/12, change of dose, abx, or anti-epileptic medication
FHx Ask about skin problems - Atopic eczema, psoriasis
SHx travel, work, social (care home)
