Question Answer
Features of aherence to medical regimes To what extent to which patients behaviour coincides with the medical or health advice why patients don't adhere ; rational choice theory 1. good reason to why treatment doesn't work, 2. side effects, 3. coast or social difficulties, 4. may not want to check if illness is still there when they stop
Bulpit; reasons for non Adherence The Aim ? To review research on adherence in hypertensive patients (high levels of stress and blood pressure)
Bulpit; reasons for non Adherence Method? A review article identifying problems with taking drugs for high blood pressure
Bulpit; reasons for non Adherence Procedure ? Research was analysed to identify physical and psychological effects of drug treatment on a persons life
Bulpit; reasons for non Adherence Finding ? There were many side effects of taking anti hypertension medication i.e. Physical reactions, sleepiness, dizziness and lack of sexual functioning they also have an impact on cognitive functioning in one study by curb 8 % males dicontined treatment due to sexual problems and 15% from medical research council
Bulpit; reasons for non Adherence conclusion ? If the coast of taking medication such as side effects outweigh the befits of the treating mainly asymptomatic problem i.e hypertension there is less likely hood of the patient adhering to it.
Bulpit; reasons for non Adherence Evaluation ? *Lack genralsibilty as only Asymptotic patients used *only males *Useful to an extent as it allows doctors to become more aware to help the patient more *Temporal validity as treatments improve over time.
Measuring of non adherence ? 1. self report ; subject to bias 2. Therapeutic outcome 3. health worker estimates 4. pill and bottle count 5.Mechanical method 6. Biochemical test ; however only can tell presence not if its taken regularly
Lustman ; Measuring non adherence the aim ? To asses the efficacy of the anti- depressant fluoxetine in treating depression by measuring glycemic control
Lustman ; Measuring non adherence Method? Randomised controlled double blined study
Lustman ; Measuring non adherence Participants ? 60 patients with type 1 or type 2 diabetes and also, diagnosed with depression
Lustman ; Measuring non adherence procedure patients were randomly assigned to taking fluoxetine or the placebo group. They were assessed for depression via psychometric test. Adherence was assessed by measuring GHB levels their glycemic control
Lustman ; Measuring non adherence findings ? Patients given Fluoxetine reported lower levels of depression and had lower levels of GHb levels thus improved adherence
Lustman ; Measuring non adherence Conclusion ? Measuring GHB in patients with diabetes indicates there level of adherence and greater adherence was shown more in those less depressed.
Lustman ; Measuring non adherence Evaluation ? *Lack genralbilty ; only diabetes patients used however this enable doctors to understand adherence in diabetes patients more *quantitative data used thus reliable however lack qualitative in depth detail *reductionist ignores therapy as it goes straight to drugs
Improving adherence methods
Improving adherence methods
Improving adherence methods
Watt ; improving adherence aim ? To see if the funhaler can improve children adherence to medication for asthma
Watt ; improving adherence Method? Quasi/ field experiment ; 2 conditions and then a self report to measure adherence rates
Watt ; improving adherence Design ? Repeated measures as each participant had one week using normal pMDI inhaler and then one week using funhaler
Watt ; improving adherence Particpents 32 Australian children 10 males, 22 females - ages 1.5 years to 6 years. All had been diagnosed with asthma. parents had also gave consent
Watt ; improving adherence Procedure ? 1st week children used normal inhaler then questionnaire given for parents to complete 2nd week ; children used fun haler and parents given another matched questionnaire. Funhaler is incentive toy and has spinner and whistle for when child takes a deep breath (positive reinforcement)
Watt ; improving adherence Finding ? 38% parents found to have medicated their children previous day when using the fun haler compared to existing
Watt ; improving adherence Conclusion ? Previous research has given reasons for non adherence in children with asthma due to boredom, forgetfulness and apathy. The fun-haler is set out to reinforce the correct usage of the inhaler toy that spins and whistles. This did improve adherence to medication by making in fun.
Watt ; improving adherence Evaluation *Cant randomly allocate groups * face valid *real life application however doesn't take in to account individual differences *Useful in helping us understand children regmine to increase adherence *Temporal validity as, children grow out of it
