1. 1st order neuron
  1. its cell bodies lie in the retina (ganglion cells)

Types of brain tumors
1. Adults tumors
  1. primary tumors are usually supratentorial.
  2. glioblastoma multiforme
    1. Malignant, high-grade tumor of astrocytes
    2. Most common primary malignant CNS tumor in adults
    3. Usually arises in the cerebral hemisphere; characteristically crosses the corpus callosum
    4. Butterfly lesion
    5. Histology
      1. Characterized by regions of necrosis surrounded by tumor cells (pseudopalisading) and endothelial cell proliferation; tumor cells are GFAP positive.
    6. Poor prognosis
  3. schwannoma
    1. Benign tumor of Schwann cells
    2. involves cranial or spinal nerves; within the cranium, most frequently involves cranial nerve VIII at the cerebellopontine angle (presents as loss of hearing and tinnitus)
    3. Tumor cells are S-100 positive.

      1. Good prognosis after resection
  4. meningioma
    1. Benign tumor of arachnoid cells
    2. Most common benign CNS tumor in adults
      1. More commonly seen in women; rare in children
    3. May present as seizures; tumor compresses, but does not invade, the cortex.
    4. Imaging reveals a round mass attached to the dura.
    5. Histology
      1. shows a whorled pattern; psammoma bodies may be present.
    6. Prognosis

2. Children tumors
  1. primary tumors are usually infra tentorial
  2. pilocytic astrocytoma
    1. Benign tumor of astrocytes
    2. Most common CNS tumor in children; usually arises in the cerebellum
    3. Imaging reveals a cystic lesion with a mural nodule
    4. Histology
      1. Biopsy shows Rosenthal fibers (thick eosinophilic processes of astrocytes) and eosinophilic granular bodies; tumor cells are GFAP positive.
    5. Grading
  3. ependymoma
    1. Malignant tumor of ependymal cells
    2. Most commonly arises in the 4th ventricle; may present with hydrocephalus
    3. Histology
      1. Perivascular pseudorosettes are a characteristic finding on biopsy
    4. Prognosis
      1. Recur after surgery, acquire more aggressiveness
        Prognosis depends on the location and its histologic grade.
  4. medulloblastoma
    1. Malignant tumor derived from the granular cells of the cerebellum (neuroectoderm)
    2. Histology
      1. Histology reveals small, round blue cells; Homer-Wright rosettes may be present.
    3. Poor prognosis; tumor grows rapidly and spreads via CSF.
      1. Metastasis to the cauda equina is termed 'drop metastasis
    4. Prognosis
      1. Resection & radiation allow 5-year survival of 75%.
3. Metastatic Brain tumors

4. Other tumors
  1. OLIGODENDROGLIOMA
    1. Malignant tumor of oligodendrocytes
    2. Imaging reveals a calcified tumor in the white matter, usually involving the frontal lobe; may present with seizures
    3. Histology
      1. 'Fried-egg' appearance of cells on biopsy
    4. prognosis
      1. Slow-growing, long survival (5-10 yrs)
      2. Grade 2 (with treatment): 10-20 years
      3. Grade 3 (with treatment): 5-10 years
      4. 5-year survival rate is 75%, 10-year survival rate is 50%.
