Hyperthyroidism
  1. usually results from Grave's disease (autoimmune thyroiditis)
    1. secondary to production of thyroid stimulating Ig (TSIs)
    2. most often seen in teenage girls
      1. clinical features
        1. similar to adults
          1. but eye signs uncommon in kids
          2. systemic
            1. anxiety, restlessness
              1. increased appetite
                1. sweating
                  1. diarrhoea
                    1. weight loss
                      1. rapid growth in height
                        1. advanced bone maturity
                          1. tremor
                            1. tachycardia, wide pulse Pa
                              1. warm, vasodilated peripheries
                                1. goitre (bruit)
                                  1. learning difficulties/behavioural probs
                                    1. psychosis
                                    2. eye signs
                                      1. exophthalmos
                                        1. ophthalmoplegia
                                          1. lid retraction
                                            1. lid lag
                                          2. Ix
                                            1. thyroxine (T4) &/or tri-iodothyronine (T3) levels elevated
                                              1. TSH levels suppressed to very low
                                                1. Antithyroid peroxismal antibodies may also be present
                                                  1. can -> spontaneous resolution of thyrotoxicosis
                                                    1. but subseq cause hypothyroidism (called hashitoxicosis)
                                                2. Mx
                                                  1. medical Rx
                                                    1. 1st line
                                                      1. drugs interfering w/ thyroid hormone synthesis
                                                        1. carbimazole
                                                          1. thyrouracil
                                                            1. risk of neutropenia
                                                              1. advise fam to seek urgent help & blood count
                                                                1. if sore throat and high fever occur on starting Rx
                                                              2. given for 2 years
                                                                1. control thyrotoxicosis
                                                                  1. eye signs may not resolve
                                                                  2. 75% relapse when meds stopped
                                                                    1. 2nd course of meds or surgery (subtotal thyroidectomy)
                                                                      1. -> permanent remission
                                                              3. beta blockers
                                                                1. symptomatic relief of
                                                                  1. tremor
                                                                    1. anxiety
                                                                      1. bradycardia
                                                                      2. initially
                                                                    2. radioiodine
                                                                      1. always follow up
                                                                        1. thryoxine replacement often needed for subseq hypothyroidism
                                                                      2. neonatal hyperthyroidism
                                                                        1. occurs in infants of mums w/ Grave's
                                                                          1. from transplancental transfer of TSIs
                                                                          2. Rx required
                                                                            1. potentially fatal
                                                                              1. but it resolves spontaneously with time
