                          1. good infection control/ hygiene, avoid dental work.
                            1. antibiotic prophylaxis for high risk women
                2. Peripartum Cardiomyopathy
                  1. rare. high mortality 25-50%
                    1. occurs in last month of pregnancy up to 5 months postpartum
                      1. sudden onset. no previous symptoms
                        1. enlargement and inflammation of myocardium
                          1. linked to myocarditis, viral infection, long term oral tocolytic therapy, cocaine abuse
                            1. treatment involves medication to decrease pulmonary congestion and fluid overload
                              1. oxygen, diuretics, vasodilators
                                1. inotrophic agents to improve myometrial contractability
                              2. left ventricular heart failure
                      2. Rheumatic Heart Disease
                        1. result of acute inflammatory complication
                          1. group A streptococcus
                            1. endocardium inflamed and scarred
                              1. mitral valve scarred. becomes distorted and dysfunctional
                                1. cusps of valves roughened
                                  1. fuse together and narrow
                                    1. mitral or atrial stenosis
                                      1. heart works harder to pump blood leading to hypertrophy
                                        1. heart increases in size
                                          1. weakens heart. pregnancy can cause failure
                            2. rare in UK. 25% of women not born in UK
