Thyroid Pharm

Kate Parvey NURSING PHARMACOLOGY Graduate English 14 cards

Cards in this deck (14)

  1. 1DRUGS USED TO TREAT HYPOTHYROIDISM?THYROID HORMONES/ SYNTHETIC THYROXINE
    1) Levothyroxine
    - synthetic T4
    2) Liothyronine
    - synthetic T4
    3) Liotrix
    - synthetic T4 & T3
  2. 2NORMAL THYROID HORMONE FUNCTIONSIncrease metabolic rate, protein synthesis, cardiac output, renal perfusion, oxygen use, body temperature, blood volume, and growth processes.
  3. 3LEVOTHYROXINE ( Use & MOAFor: HYPOthyroidism
    Thyroid hormone replacement therapy
MOA: most is converted to T3 in body. Thus can produce nearly normal levels of both T3 & T4
  4. 4LEVOTHYROXINE AE & MonitoringAE: can cause cardiac stimulation
    Monitor: T4 & TSH levels
    s/s of hypo/hyperthyroidism
    * when taken correctly rarely causes AE
  5. 5LEVOTHYROXINE Drug Interactions & Nursing ConsiderationsnNarrow therapeutic index,
    Long half life (~7days)
    - b/c highly protein bound
    - slow to "kick in", ~ 1mo to reach steady state
    Drug-Drug Interactions:
many can reduce absorption and accelerate metabolism
  6. 6WHAT TREATS HYPERTHYROIDISM?ANTI-THYROID DRUGS= THIONIMIDES
    1) Propylthiouracil
    2) Methimazole
  7. 7PROPYLTHIOURICAL Uses & MOAFor: Graves disease & HYPERthyroidism
MOA: suppress/block synthesis of thyroid hormones.
Interferes with Iodine incorporation AND can inhibit peripheral DE-iodination of T4 to T3
  8. 8PROPYLTHIOURACIL AE & Nursing ConsiderationsAE: Liver injury, agranulocytosis, rash
NC: safer ( than methimazole) for use in women who are pregnant or breastfeeding ; doesn’t cross the placenta easily b/c protein bound
  9. 9METHIMAZOLE Uses & MOAFor: Graves disease & HYPERthyroidism
MOA: suppress/block synthesis of thyroid hormones.
    Interferes with Iodine incorporation
  10. 10METHIMAZOLE AE & Nursing ConsiderationsAE: Agranulocytosis= increased risk of infection
    Neonatal hypothyroidism in pregnancy
*Not rec in pregnancy- breast milk and transplacental passage.
  11. 11WHAT ARE 2 COMMON E IODIDE PRODUCTS ?1) Lugols Solution
Strong iodine solution of 5% elemental iodine and 10% potassium iodide
    5-8 drops Q6hr
    2) Saturated Solution of Potassium Iodide (SSKI)
    6 drops Q6hr
  12. 12POTASSIUM IODID Uses & MOAFor: Acute treatment of HYPERthyroidism
MOA: creates such high levels of iodide that it will:
    - reduce iodine uptake by thyroid gland
    - inhibit thyroid hormone production
    - block release of thyroid hormones into bloodstream
  13. 13POTASSIUM IODIDIE AE & PrecautionsAE: Iodism= brassy taste in mouth, burning sensation in mouth, sore teeth.
Precautions: use of potassium-sparing diuretics, potassium supplements, and ACE inhibitors increases the risk of hyperkalemia.
-intake of foods high in iodine
  14. 14EVALUATION OF DESIRED EFFECTS- Decreased T4 levels
    - Reduction in size of thyroid gland
    - improved sleep, weight gain; blood pressure and heart rate w/in range
