1. What is it?

   • Set of issues or questions that must be answered in an exam, test, quiz, survey, etc.

2. What is it for?

   • They can be used as data collection instruments, or as an evaluation tool in the school setting

3. How is it done?

   • It consists of a series of questions to be asked and instructions for their completion, both for the interviewer and the respondent

4. What are the types of questions contained in the questionnaire?

   • They are classified as open, closed and mixed: Open questions are those in which after the statement there is a blank space for the respondent to answer what he or she wants. Closed questions are those in which the interviewer establishes all the possible answers to the question

5. What should the first questions of a questionnaire be?

   • Clear and simple initial instructions (how to respond in general, with examples if required). In some cases it is convenient to start with neutral or easy to answer questions. It is not recommended to start with difficult or very direct questions
