1. Molecular biology

   • 1. Gen in het genoom traceren
     • 2. AZ volgorde achterhalen
       • 3. Polymerase chain reaction: begin en eind stukje van gen door PCR laten lopen en het gen is te pakken
         • 4. Restrictie enzymen knippen het gen, zodat het past in plasmide
           • 5. Plasmide in bacterie -> produceert gewenste stofje
   • copy DNA libraries
     • cDNA gemaakt uit RNA mbv reverse transcriptase
       • Middels hybridisatie met een gelabelde probe wordt de gen van interesse eruit gevist
         • cDNA in bacterie -> maakt gewenste stofje (bv. insuline) aan
   • Genetic engyneering
     • EPO kan niet door bacteriën worden afgebroken, omdat het geglycosyleerd is. Patiënten met te weinig rode bloedcellen of die een operatie ondergaan, hebben hier baat bij.
       • EPO =humane erytropoietine

2. Monoclonal AntiBodies (-mab)

   • Herkomst
     • Chimere AB: menselijk AB met Fv van dier (-ximab)
       • AB dier = lichaamsvreemd voor de mens en wekt dus een immuunrespons op
       • Fv (=uiteinde) dient ter herkenning van antigen/epitoom
       • Infliximab remt ontsteking (ziekte van Crohn = TNF-a teveel) door TNF-a aan R binding te blokkeren.
     • Volledig humane AB (-umab)
     • Gehumaniseerd (-zumab)
       • Herceptin (transtuzumab) wordt gebruikt bij borstkanker: overproductie HER2-R
         • Bestaat uit 2 delen: herkent kankercel en labelt deze ter herkenning door de immuuncellen.
     • Van dier: bv muis (-omab)

         • Replacement: mimic the normal function of a protein
         • 'Antagonist': protein function is inhibited - usually Ab
       • Limitaties
         • Variatie in eiwitten en ongeoptimaliseerde fysico-chemische eigenschappen

         • Deficiëntie in de farmacokinetiek

       • Twee biologisch vergelijkbare gnm, maar niet identiek. Variatie kan ontstaan tijdens het productieproces.
       • - Biosimilars are not generics, also they are not original biological drugs, but they represent novel third group of advanced biological drugs! - Biosimilars should pass the extremely strong criteria of quality, efficacy and safety by means of CLINICAL TRIALS! (contrary to generics that are NOT applied to clinical testing!)!
       • Down stream processing

       • Additieven: oplosbaarheid, absorptie, buffer, conservatie, anti-oxidantia, stabilisatoren voor lyofilisatie
