Nitroglycerin: CONTRAINDICATIONS Increased intracranial pressure, severe anemia, cardiomyopathy, (caution: severe renal or hepatic disease, acute MI, severe hypotension, hypovolemia, pregnancy, breastfeeding
Nitroglycerin: INTERACTIONS Increased effect w/ alcohol, beta blockers, calc-channel blockers, antihypertensives, aspirin, benzos, vasodilators,Decreases effects of heparin; Herbs: hawthorn increases nitro levels
Nitroglycerin: ASSESSMENT -Obtain baseline vitals -Obtain health and drug histories because Nitroglycerin is contraindicated for hypotension or acute myocardial infarction
Nitroglycerin: INTERVENTIONS -monitor vital signs -position patient sitting or lying down when taking 1st nitrate -offer sips of water before giving Sublingual nitrates (dryness inhibits absorption) -monitor effects of IV nitroglycerin -Apply ointment without using fingers (use gloves) -Do not apply ointment on chest
Nitroglycerin: PATIENT TEACHING -admin tablet if chest pain occurs -don't ingest alcohol -notify pcp if chest pain is not alleviated -don't use beta blockers and cal blockers without pcp approval -tablet is placed under tongue for quick absorptioin -store away from light and keep dry -apply patch 1x daily; rotate skin sites -suggest acetaminophen for relief of headache
