      1. Each individual has different needs so an assessment ensure the correct and most suitable care is provided
        1. Carried out by 'Care Manager'

        1. Set of services required for the care

          1. E.g. Social Workers / Community Nurse; manages care package, decides budget, finds people and organisation to provide service and carries out regular reviews
          2. Care Plan
            1. Record of the care expected to be received - signed by care recipient as acknowledgement that it has been agreed
          3. MEANS TESTING
            1. If someone qualifies for home social care, income is assessed to see how much they need to pay
            2. INCOME AND WEALTH
              1. Savings above £21,000 would result in needing to pay towards costs of care
              2. POSTCODE LOTTERY
                1. What you pay will depend on where you live
                2. HEALTH CARE OR SOCIAL CARE?
