    1. Acute pyelonephritis

          1. Yellow ulcers in medulla, red/oedematous pelvic mucosa, pus
            1. Polymorphic infiltrate, abscesses, ulceration of pelvis mucosa
            2. Non-obstructive
              1. Suppurative
                1. Staph aureus 2o to osteomyelitis
                  1. Enlarged, hyperaemic, yellow, abscesses in cortex
              2. Necrotising papillitis
                1. Diabetes, analgesics
                  1. Yellow/white friable necrosis in papillae
                    1. Pale infarcted papillae, haemorrhagic border, polymorphic infiltrate
                    2. Chronic pyelonephritis
                      1. Analgesic nephropathy (phenacetin)
                        1. Damage to vascular wall --> ischaemia
                        2. Hypertension, proteinuria, pyuria, haematura
                          1. No bacteria found (usually)
                            1. Atrophic/contracted, scars on cortex, dilated pelvis/calyces, sup & inf pole most affected
                              1. Hyalinised glomeruli, periglomerular necrosis, colloid casts in dilated tubules, interstital fibrosis, lymphocytic infiltrate, arteriosclerosis
                              2. Xanthogranulomatous nephritis
                                1. Proteus species, mimics renal cell carcinoma
                                  1. Enlarged, yellow nodules
                                    1. Vacuolated cytoplasm (foam cells), granulomas, giant cells, micro-abscesses
                                    2. Renal tuburculosis
                                      1. 5% of active pulmonary TB
                                        1. Lesion in medulla --> caseous foci in medulla and cortex --> destruction of kidney
                                          1. Caseating necrosis
                                            1. --> fibrosis/obstruction of ureter, cystitis, epididymitis
                                          2. Acute interstitial nephritis
                                            1. B-haem strep, diptheria, leptospirosis, filariasis, sulphonamides, penicillin, methicillin
                                              1. Swollen, red/yellow, dark red medulla
                                                1. Tubular epithelial degeneration, haem and bile casts in lumens, inflammatory infiltrate
                                                2. CMV
                                                  1. Drug Induced acute: patchy necrosis --> NSAIDs, penicillin, sulphonamides
                                                    1. Ischaemic acute tubular necrosis: sepsis, shock, haemoglobulinuria, crush injury
                                                      1. Cell necrosis, dilated tubules, interstitial oedema
                                                      2. Myeloma kidney
                                                        1. Pale, waxy, enlarged
                                                          1. DCT blocked by eosinophilic Bence Jones protein casts, amyloid in glomeruli, dystrophic calcification, tubular epithelial cells with multiple nuclei
                                                            1. Tubular damage & obstruction, nephrotic syndrome
                                                            2. Toxic acute: antibiotics, heavy metals, organic solvents, cyclosporine A
